Concierge Primary Care Membership (Premier Plan)

The Premier Concierge Membership at SASC Health & Wellness is designed for women (and men) who want more than basic primary care. This plan adds ongoing wellness and hormone/metabolic support to a concierge model that offers time, access, and—when appropriate—in-home visits within our service area.

For $250/month, you receive:

One extended annual concierge wellness & optimization visit
A comprehensive visit that looks at your health from the inside out—labs, hormones (when appropriate), metabolic health, lifestyle, and long-term goals—so we can create a tailored plan, not a one-size-fits-all approach.
Quarterly wellness/optimization visits
Up to two (2) additional wellness or optimization visits per year (in-office or telehealth) to monitor progress, fine-tune your plan, adjust medications, and keep you on track.
Up to 2 sick/acute visits per month for new issues
Access for non-emergency problems such as infections, symptom flares, new concerns, or medication side effects.
Important: Limited to two (2) sick visits per calendar month for new complaints. Unused visits do not roll over, and any additional sick visits are billed at the current member sick-visit rate.
In-home visit option (Premier members only)
When clinically appropriate and within our practice service area, select visits may be provided in your home (for example, certain wellness, follow-up, or non-emergent visits). In-home scheduling is based on safety, location, and provider availability, and is offered at the discretion of SASC Health & Wellness.
Priority and enhanced access
- Priority same or next-business-day appointments when available
- Priority access to limited evening/weekend slots when offered
- Enhanced secure messaging for brief, non-urgent check-ins between visits during business hours
Preferred pricing on select services
Premier members receive preferred member pricing on eligible wellness therapies and other services listed on the SASC Health & Wellness fee schedule.

Important notes

The Premier Concierge Membership is not health insurance and does not replace emergency care, specialist care, or hospital services.
You are encouraged to maintain separate medical insurance for emergencies, hospitalizations, imaging, and specialist visits outside of SASC Health & Wellness.
This membership does not provide unlimited visits. Sick visits are capped at two per month for new complaints, with no rollover; additional visits are billed separately at the current member rate.
